As an accredited continuing care retirement community (CCRC), Acacia Creek is able to offer several levels of care, all within a few steps of your front door. That means that no matter what your or your partner’s health needs, you’ll be able to access the right level of care for you without needing to leave campus.
Independent Living
Residents entering Acacia Creek typically live independently, in their own apartment. Independent Living residents have access to a full range of lifestyle amenities, including restaurant-style dining, salon, swimming pool, and more.
Assisted Living
For residents who require assistance with daily activities, our caregivers provide discreet, in-home support with tasks such as bathing, dressing, housekeeping, medication management, personal care, and mobility. Each resident develops a care plan with a health and wellness manager. Care levels may be charged as additional services.
Skilled Nursing
For residents in need of more advanced nursing care, around-the-clock skilled nursing is provided through the Masonic Homes of California, just steps away on the same campus. The MHC skilled nursing and rehabilitation services have repeatedly earned a coveted five-star rating from Medicare. Skilled nursing is available on both a permanent basis or temporarily through the Transitions rehabilitation program for residents following surgery, illness, or injury.
Memory Care
Expert care for those experiencing memory-loss conditions including Alzheimer’s and dementia is available through the brand-new Pavilion at the Masonic Homes, located directly adjacent to Acacia Creek—meaning a loved one never needs to be separated in order to receive the expert care they need.
